You have some gameport modules loaded for your soundblastercard, but as the joypad is attached via usb, they should not be used.

try to type
modprobe usbhid
modprobe uhci_hcd
though I fear that won't change anything.

fine.
If you like, you can add these commands in
/usr/local/Joystick-sidewinder/Joystick-sidewinder

Then you can activate your joypad with a mouseclick.

Or to activate it when Puppy boots, add them to
/etc/rc.d/rc.local :)
